Your proof of immunizations must be on a Florida Department of Health 680 blue card. Contact your local healthcare provider or the Orange County Health Department to obtain a card for your child.

You must provide proof of well-child physical exam within the last year to register in OCPS. Free exams are available from Pediatric Nurse Practitioners at the Student Health Center at Glenridge Middle School. Call 407-623-1415, ext 256 for an appointment.
